I am Dr. Tom Castellano and I am currently practicing in my home state of Colorado in the field of Emergency Medicine and Geriatric Medicine. I am offering seminars on end of life decisions. This original lecture was conceived out of my love of teaching and peoples desires for more information about how to plan for their end of life.

Whether for your own planning, or for those decisions you would help make on your loved one’s behalf, this program will define terms, guide you through the needed papers and give you information on how to best protect your values and preferences. I am offering my class on DVD which then can be completed at your own pace. This program includes educational samples of many types of Advance Directives; the papers that help communicate your wishes when you cannot speak for yourself. (Your primary care provider and your estate lawyer will guide you through the exact needs of the statutes and policies of your state.)

This DVD course covers what you will need to know about planning and documenting your decisions and wishes. I include statistics and scientific knowledge combined with a sensitive approach to difficult subjects of death, resuscitation, illness and aging. The companion papers accompanying the DVD can be filled out and shared with your lawyer and primary care provider.
